9/22/2023 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, Josiah and his mother were taking turns making each other laugh. The family appeared to be in a good mood and ready to implement the strategies provided by QMHP-C. Josiah appeared to be in good spirit and happy as evidenced by him smiling and engaging in conversation with counselor about his day. Mother was relaxed and happy as evidenced by her appearance. Mother and Josiah were on the same page and displaying a positive relationship with one another. Behavior: During the second portion of the session, Josiah was a little withdrawn from the session as he was paying attention to what his other sibling was engaging in. After redirection, he was able to regain focus on the session. Intervention: QMHP-C processed with Josiah the feelings he shared and assisted him in sorting through them. QMHP-C assisted Josiah in understanding the anger that he feels and what he believes causes him to react the way he does. QMHP-C assisted Josiah in going through each step of the problem, the cause, the reaction and solution of each situation. QMHP-C encouraged Josiah to continue to work on understanding his feelings and making sure that he responds in a positive manner. QMHP-C will explore positive self-talk, use instruction modeling, and increase social communication skills with positive reinforcement role play activities. QMHP-C will educate Josiah on behavior modification to help Josiah identify what behavior he needs to decrease and avoid daily. QMHP-C encouraged Josiah to utilize positive self-talk as a means of decreasing anxiety and managing frustration when encountering difficult or challenging circumstances. QMHP-C talked to Josiah about his confidence levels and how they can affect his day and self-esteem. QMHP-C encouraged Josiah to make good choices. QMHP-C monitored acquisition of behaviors and managing individualized behavioral modification programs. QMHP-C provided simple praises to Josiah in efforts to motivate Josiah to have a good rest of the day. QMHP-C encouraged mom to provide frequent positive reinforcement to maintain Josiahâs interest and motivation in maintaining positive behaviors and completing his virtual work assignments. QMHP-C reviewed the reported behaviors with Josiah in efforts to allow him to express his point of view and self-reflect. QMHP-C did not have to prompt or intervene any negative behaviors during the session this evening. Response: Josiah communicated his likes and dislikes. Josiah recognized his triggers and was able to identify common interests and similarities him and other ADHD recipients share. Josiah communicated by nodding and exhibiting good eye contact. Josiah verbalized positive feelings about goals and objectives. Josiah communicated that he wants to work independently. Josiah displayed good participation in various settings and was responsive to all instructions. Josiah understands that he is expected to act civilized and treat others with respect. Josiah responds well to praise, support, and positive reinforcement provided by QMHP-C and is seen increasing self-esteem and confidence. Josiah said he was going to practice coping strategies and relaxation techniques in all aspects of his daily living routine in hopes that he will be a better son, brother, and friend. Josiah was able to cooperate and comply to rules and regulations due to positive peer influence. Josiah implemented pro social skills successfully. Josiah attends to his treatment 4x a week and has completed all psycho educational activities with QMHP-C, as observed. Josiah has presented himself in a responsible way and is making progress in all areas. Josiah has become more involved in age appropriate social activities and was given positive reinforcement for these choices. Josiah demonstrated expectations and responded well to feedback. Josiah reported a decreased stress level due to his modulation of social contact, and the success of this was evaluated.
